Planning your visit at the kidney clinic

What to bring to your :

Your insurance card/cards and your ID.

All of your home medications in a bag including supplements and over the counter medications.

A New Patient Packet will need to be completed during your first visit, you can download and print the forms and bring the completed forms with you to the appointment.

Your discharge instructions from the hospital if any.

A family member and/or friend to support you if needed.
